Referring a patient to our facility

Patients may be referred to us either by a physician, case manager, family member or other health care professional. At the time of referral, our Admissions Office will ask for specific information about the patient's illness or injury, insurance information, and who will be the point of contact.

Our pre-admissions review team will review the patient's medical history to determine if the patient meets the criteria for admission. We must obtain the medical records from the referring physician or hospital rather than records provided by family members. Please provide the discharge planner authorization to forward records to MedStar NRH.

A rehabilitation nurse liaison will pre-screen the patient at the acute-care hospital and answer any questions that might arise. Our nurse liaison is an active member of the MedStar NRH Admissions team and provides a critical link to the acute-care hospital's discharge planning team.

INPATIENT - Referrals may be phoned into our Admissions office by calling 202.877.1152. Our fax number is 202.722.4432.

In order to help facilitate the work of the case manager or discharge planner, MedStar NRH also participates in e-Discharge.
